Sports Physical Forms:
A sports physical must be on file in the health office in order for your child to participate in athletics.

Medication Administration Policy:
Written authorization from both parent/guardian AND physician/licensed prescriber must be received before ANY medication can be administered. It needs to be renewed annually or whenever the medication changes.

Reminder for Families of incoming 7th graders!
No Shots, No School!
State law requires students entering 7th grade to have the following shots:
MMR: (Measles, Mumps & Rubella)-Two doses given after the first birthday;
Td/Tdap: (Tetanus, Diphtheria & Pertussis): One dose given after 11th birthday;
Hepatitis B: A series of three shots given over a six-month period;
Varicella: Two doses given after the first birthday, or date of disease;
Polio: At least three doses. |
